MartinLift – The Lunchtime Facelift

If you’re not ready for a full-on facelift or just unable to deal with it all, there
is a procedure you called the MartinLift.

For the past decade and a half, we have worked to perfect the placement
of “smart thread” in strategic areas of the face to lift target areas.
The most common are is the cheeks. There, the fat pad that we started
with has moved south by the time we reach forty and we need to lift it back
up and get rid of the sag and the jowls.

The procedure is done under local anesthesia with an accompanying
injection of Demerol for pain control. Areas on the face are mapped out and
lightly marked. After the skin is cleansed, local anesthetic is placed along
the lines where the smart threads will go.

The threads are carried into the targeted areas with a needle and once the
needle is removed tiny barbs open up on the edges of the smart threads to
lock them in place. If you think about a staple with two barbs, you can
imagine how well a hundred barbs holds.

After the procedure, we have very little swelling. There is little or no post-op
pain and no drainage tubes as with common facelifts.

The ideal patient for the procedure is a person who is fit and within 5-10
pounds of his ideal weight.

MetroMD Summer Body Liposuction

As summer approaches, and a half year’s worth of work is about to be displayed at pools, parties, and events, one must now evaluate if they have truly obtained their ideal summer body.

At MetroMD, our approach to liposuction not only allows for in-office treatment without general anesthesia, but the surgical approach includes pre and post-operative optimization to enhance a patient’s overall potential to achieve the results they desire. Areas often treated for a slight tune up may include: • Abdomen • Waist (“Love Handles”) • Posterior Arms (“saggy arms”) • Posterior flank (“bra bulge”)

MetroMD offers a unique innovation on traditional liposuction, Tumescent Smart Laser Liposuction, in which a combination of laser wavelengths allows for enhanced adipose cell breakdown increasing ease of fatty tissue removal, decreased bleeding and bruising, and the promotion of neocollagensis enhancing the overall degree of skin tightening post-procedure. Additionally, the use of tumescent anesthetic solution (combination of Lactated Ringers, lidocaine, epinephrine) allows for the patient to be awake and with slight sedation throughout the procedure with minimal if any discomfort whatsoever.

Depending on the treatment area, typical recovery time is anywhere from 3-6 months with the maximal effect typically visible around the 6 month time point. Additional therapies such as cool sculpting are always recommended as adjutant therapies to improve likelihood of success post-procedurally.

The final component of the MetroMD approach with lipolytic therapy is inclusion of pre and post operative physiologic optimization practices such components of the Enhanced Recovery After Surgery (ERAS) program that is growing in favor in hospitals world-wide. The goals of enhanced recovery after surgery (ERAS) protocols include attenuating the surgical stress response and reducing dysfunction through integrated preoperative, intraoperative, and postoperative pathways. In brief, any surgical procedure, cosmetic liposuction included, is akin to running a marathon or greater with regards to the metabolic demand placed on the body to heal. If one is not prepared for this endeavor, it should be no surprise optimal results are rarely achieved.

MetroMD CO2 Laser Therapy for Smoker’s Lines

A common complaint from many patient’s who present is the desire to improve appearance over time of vertical lip lines, commonly referred to as “smoker’s lines” or “lipstick lines”. These wrinkles are caused by a variety of factors, including age, genetics, repetitive lifestyle habits ( i.e. drinking straw/bottle use), and in some individuals sequelae from long periods of smoking.

Beyond topical collagen repair serums, diligent sunscreen utilization, and lifestyle modification, one of the very effective treatments that can be used for these troubling areas can be CO2 Fractional Laser Therapy.

With the technological advent of fractional CO2 laser resurfacing (developed in the 2000’s), a novel mechanism in which numerous narrow, microscopic columns of laser light are emitted was created. This then allows for focused and precise treatment of a defined fraction of the skin within a targeted area, leaving intervening areas of skin between the columns unaffected. The combination of damaged and undamaged skin adjacent to sites of laser injury allows for rapid reepithelialization of tissue after treatment through the migration of viable cells into wounded areas lifting, tightening, and regenerating healthy and youthful appearing skin.

Typical treatment time, including pre-procedural preparation takes approximately 45 minutes. Recovery time can be anywhere from 3-7 days depending on the patient’s individual presentation and full effect is best observed 6-8 weeks following procedure. Most individuals require 2-3 treatments for maximal result.

Complications and adverse effects are rare but may include persistent redness, dyspigmentation, infections, and scarring. Transient postinflammatory hyperpigmentation may occur in approximately 30 percent of patients and is more likely to occur in patients darker skin pigmentation. YAG Laser Freckle Removal at MetroMD in Los Angeles

With the summer months coming freckles become more prominent than ever.  The extra sun exposure tends to make these small spots more evident. Many of our patients come to us looking for solutions to eliminate these spots. One of our favorite modalities for removing freckles is using YAG laser technology.

YAG laser for freckle removal is a great treatment. This laser simply attracts towards the pigment on the skin and breaks it down. This same laser is used for tattoo removal and the mechanism is similar. The laser penetrates the skin and hits this pigment and breaks it up. Over time your skin absorbs this pigment and some falls off.  This then leaves your skin clear of freckles.

Fractional CO2 Laser utilization for Non-Invasive Eye Lift

Originally performed in the 1980’s, continuous wave carbon dioxide (CO2) laser has been used to treat cutaneous signs of photo damage that progressively increase with age and occur most prominently in individuals with fair skin.

With the technological advent of fractional CO2 laser resurfacing (developed in the 2000’s), a novel mechanism in which numerous narrow, microscopic columns of laser light are emitted was created. This then allows for focused and precise treatment of a defined fraction of the skin within a targeted area, leaving intervening areas of skin between the columns unaffected. Botox to Smooth out Fine Wrinkles

Botulinum toxin, or Botox, in brief, is a neuromodulator derived from neurotoxins produced by Clostridium botulinumInitially procured for use for medical conditions from migraine therapy, hyperhydrosis and others,Botox has recently gained wild popularity for cosmetic use when injected with the intended purpose of paralyzing skeletal muscle.

Stem Cell Therapy for Shoulder Injuries

With the advances in cellular medicine, stem cell therapy is now available to treat soft tissue injuries to the shoulder. The MRI is critical in determining whether or not a patient is a candidate for stem cell treatment. If there is partial tearing of cartilage, ligaments and tendons, stem cell therapy offers an alternate to extended periods of physiotherapy and arthroscopic treatment in many cases.

Commercial stem cells are now available and approved to be use in such procedures. The patient’s own stem cells may also be harvested in a painless extraction of marrow from the hip. The patient is made comfortable with pain medication and the targeted area of the shoulder injected with stem cells following the guide to the pathology set out by the MRI.

Patients are requested to limit use of the treated joint for two to four weeks to enable the stem cells to adhere to the damaged tissue and not to be driven out into the peripheral circulation. The procedure is sometimes repeated within six months if more treatment is required but most patients state a single treatment is sufficient.
